I have been using a TAP3 appliance for 6 months.  I have had a sleep study titration and it solves the OSA issue.  It is hurting my jaw (yes I have done the AM exercises) and it is causing me to brux to the point that my teeth ache.  My dentist is recommending switching devices to either a SomnoDent or a Silencer.  The pictures make the SomnoDent look massive.  Does anyone using SomnoDent have difficulty with adapting to the size of it?  My dentist feels that not having the jaw locked closed, as with the TAP appliance, that I may have less problems with bruxism (which does not seem to be an issue without the appliance).  Does anyone have any comparison between any of these three devices?  They seem to be the ones that my dentist prefers, however, he does not have to wear one!!  T hank you for any input you can provide.

I have been using the Somnodent (formally Somnomed) for about four weeks now.  I have had no major problems as of yet other than a very slight chafing on inner cheek from the vertical fins on the lower jaw appliance that has since gone away.  When I first put the appliance in I have a slight tightness in the jaw joint until the jaw moves forward.  I can relieve this by opening and closing my mouth a few times.  In the morning I have no pressure at all.   This is one of the reasons I went with the Somnodent instead of the TAP.  I use the bite tabs and lean on my chin to move the lower jaw back in the morning. My doctor believes I have been adjusted out far enough. ( 3.5 mm from the median start point. I believe I am out at least 1/4 inch from my "normal bite" position)  We have based this upon how I have felt in the morning and during the day. I have an almost completely eliminated the snoring.  I will have a sleep study this week to confirm what I hope to be improved AHI and O2 levels.  I say improved as I was at 49.7 AHI.  Good Luck to you.
